The ‘Level Foundation’ Principle in Chiropractic

The Gonstead System of Chiropractic

Dr Clarence S Gonstead developed his system the Gonstead System over many years. He used results and repetition to guide the systems development.

A key principle he put forward and developed was the 'level foundation' principle which is parallel in the engineering industry. It revolves around the fact that the pelvis (Ilium and sacrum) forms a ring structure that supports the column column. This 'foundation' if unlevel or tilted away from a 'normal' position sets the vertebral column into a compensatory deviation. This is to try and return the eyes to a horizontal plane because this is a normal compensation that is required for ambulation and erect posture.

The pelvis

The pelvic girdle consist of the left and right Ilium. They are joined at the front (pubic symphysis) by a flexible joint called a diastasis joint. The rear of both Ilium is separated by the sacrum which is an inverted triangular bone and is joined to each Ilium by a diarthrosis joint called the Sacroiliac joint or SI for short.

The SI joint put simply has an L-shape and the bottom part of the joint allows mostly a pivoting movement where the top part allows a small amount of gliding and has a ridged surface. The whole joint is held together by ligaments. It provides a low friction environment that can withstand a great amount of load. While the motion of this joint was document as early as the mid 1800's it was not readily accepted until the early 1900's.

Because the SI joint allows for motion and has a ridged surface held together by strong ligaments any disruption to its position can impede motion. Since it is a ring structure any restriction in normal position will therefore affect the whole pelvic girdle.

The concept of the level foundation suggests that there will be a need to compensate for any restriction or change in the normal position or motion of the pelvis. This compensation can be evident in the opposition SI joint, the pubic symphysis or the vertebral column supported by the pelvic.

With a short leg the pelvis will lean to the short leg side and the compensation for that can be in the pelvis or just the vertebral column. However, the low side SI joint will be loaded differently to the high side.

How this impacts the SI joint motion is controversial. Some practitioners challenge the idea that the pelvis can be restricted. As a result, views vary and practitioners use diverse approaches to help patients.

Chiropractors have a long history of using 'spinographs' (full spine standing X-rays) to confirm any foundational changes. Spinographs reflecting corresponding payments elsewhere in the spell when a distortion is in the pelvis.

It must be said that the motion of the SI joints can not be assessed by standard X-rays. So Chiropractors compare relative motion of each SI joint to decipher any positional abnormality that corresponds to the motion change. Chiropractors adjust restricted SI joints to return normal positioning and therefore normal motion.

This is a very useful non-invasive approach in the effective care of patients with Sacroiliac dysfunction and other associated symptoms. This systematic assessment of the foundation to the spine is the method used in the Gonstead System of chiropractic.

Disc Replacement or Spinal Fusion: What’s the Best Option?

Back pain is one of the most common health problems people have to face at one point or another in their lives. It is estimated that about 75% of the world's population will experience lower-back pain at one point in their lives. While most people use different types of medications to alleviate their pain, some need to get under the knife to get rid of their back complications. Back surgery is considered to be a big deal as it is not only very complex, but can also take a minimum of 3 to 4 hours or more to complete. It is only recommended if a person finds it difficult to perform daily tasks and has chronic pain regularly getting non-invasive treatment.

There are two types of back surgeries that your doctor may prescribe, namely – artificial disc replacement (ADR) and spinal fusion. While both are advanced ways of treating back pain, both have their own disadvantages. In order to replace your problematic disc, the surgeon would reach your lower back from the front as this would allow the surgeon to access your spine by moving the blood vessels to the sides, taking precaution as to not offset vital and sensitive nerves. This procedure would require more than one surgeon and would take about a minimum of 2-3 hours. Before the surgery, the doctors would have to do a number of x-rays and determine exactly which disc has to be removed. Once the anesthesia puts the patient to sleep, the doctors would remove the disc and insert an artificial one.

On the other hand, spinal fusion does not require any external / artificial bone or disc. In fact, it involves a “welding” process. A joint is welded between the most painful vertebrae – which became a single bone after the patient suffering from back pain has recovered and the bone has cured.

Spinal fusion can be done using different techniques. Your surgeon can approach the vertebrae from where it is most suitable. He / she can cut through the patient's front, back or even both depending on the condition and requirements. This surgery may take up to 5 hours.

It is strenuous to decide which surgery to go for as there are so many factors involved. A number of cases have been reported where patients who chose spinal fusion still experienced back pain. While this might be the outcome of a poor after-surgery care, some say spiral fusion tend to fail most of the time.

There have been very few cases reported about failed disc replacement surgeries and most people who have already gone through this surgery say this has actually improved their back pain and allowed them to get back to their normal lifestyle. The Best Experience of Pain Ever

I just have to share my recent experience of pain – mostly because it was SO DIFFERENT than what I have experienced in the past.

A few days ago as I was bending over doing housework, I felt a tweak in my low back. It felt like something had caught and gotten stuck and I was not entirely sure I could stand back up. I could feel a little fear rising in my chest because this particular tweak had never happened before. I took a couple deep breaths and managed to stand up, but my back still felt tight, twisted and just plain “off.” I rated the pain a 7 out of 10.

In the past I would have immediately gone to fear. Oh my God! Something's horribly WRONG !! How long is this going to last? How long am I going to be out of my activities? Am I going to be STUCK like this FOREVER? !! And on and on … In the past, tweaks like this would get worse, pain would spread through my body and I might be miserable and in pain for several months.

As an EFT / Tapping practitioner, I now understand pain as “stuck energy” and fear as the largest magnifier of stuck energy, so I immediately began tapping.

Even though that was a little shocking and scary and this does not feel good, I know this is going to be OK.

My fear came right down and my pain level came down a notch or two as well. I have a wonderful chiropractor who I trust, and I was able to get an appointment three days later. In the past three days would have seemed an eternity, but this time I told myself it was an opportunity for me to fully experience tapping for pain.

While I had been in chronic pain for several years, I had not had any real pain since I had become an EFT practitioner. My trusted chiropractor had led me out of pain originally, but I used to visit him often – first every week, then every couple of weeks. Once I began tapping, however, my adjustments held longer and longer and now I only see him a few times a year for a “tune up.”

That evening my pain was still at a 5, so I gave myself a full tapping session. It did not take long to connect this pain with my fears concerning finances. I worked with that until I felt much better, and, when I checked in with my back it was about a 2! I could still feel the misalignment, but my experience of the pain was completely different. It was a niggling feeling of being “off,” but something I could easily live with for the next couple days. This happens for my clients all the time, but I have to admit, I was still a bit amazed at experiencing such a dramatic difference for myself.

When I visited my chiropractor, he said there was a rotation in my L-4 that would definitely have caused the pain I had described. He snapped everything back in place, along with some adjustments to my neck, hips and feet until I was back in alignment again.

Even though pain is never fun, this was the best experience of pain ever! I am still on a high marveling at the power of the human mind to magnify or decrease our experience of pain and of EFT to communicate with the part of the mind that controls this experience. While I Definitely had a physical misalignment that needed attention, my thoughts, emotions, and energy flow turned out to be a larger component in my experience of pain.

Symptoms of Spinal Subluxation

The vertebral subluxation is caused due to improper position or motion of the spinal bones that upset the nerve communication between the body and the brain. The spinal subluxation is a response to stress. The misalignment of the spine could occur due to a number of reasons and its impact generally varies from person to person. Aching Back And Neck

The Importance of Movement for a Healthy Spine

Yep, your spell is incredible, alright. Designed to allow for optimal mobility while still protecting your central nervous system, your amazing back bone is nothing short of a biological masterpiece! But it can only survive with your help.

Back pain will affect around 80% of us at some point during our lives; and lower back pain accounts for more lost hours than any other type of occupational injury. So the human spine tends to get a lot of stick! But a lot of the time, it's our habits that are to blame for our back issues; and more specifically, how we tend to neglect the needs of our spines.

The problem is that we do not always use our spines in the way that nature intended. Poor postures, prolonged sitting andactivity are behaviors that our spines absolutely hate! And they throw these bad habits right back in our faces in the form of back pain.

Your spine is made up of 33 bones stacked on top of one another. The 4 bones of the coccyx are fused, as are the 4 bones of the sacrum. In between all of the other bones have two little joints called facet joints. These joints facilitate movement at each spinal segment.

We are born with fully functional and healthy spines but as life progresses, our movement tend to diminish. Of course, this has traditionally occurred with age, but these days, even some younger generations are guilty of living somewhat sedentary lives. In fact, modern day life can lead to all kinds of muscle imbalances around the spine, including tightness and weakness. Experts are saying that “sitting is the new smoking”; and they're not exaggerating! When you sit down, intervertebral discs absorb fluid and become tighter, allowing less range of motion and promoting injury. Your body simply was not designed to sit for long periods of time, so it will inevitably suffer as a result of doing so. Prolonged postures commonly lead to weakening and lengthening or tightening and shortening of muscles. The end result is often reduced movement and pain.

The facts are simple; the more you purposely move your spine, the better range of motion you will have and the less chance you'll have developing back pain. Ensuring your spinal gets the movement it needs will enable it to get the nutrients it requires to stay healthy.

TLSO Back Brace

The initials TLSO are an acronym that means Thoracolumbar Sacral Orthosis. It is used primarily for children who have scoliosis. This medical condition is a deformity of the child's spine, which causes a sideways C- or S-shaped curvature of their spine. As many as twenty-five percent have scoliosis but only about three percent have the most different form. The C-curvature is more common of the two curvatures. The S-curvature will typically form when the spine tries to correct its own from the C-curvature deviation.

The back brace fits over the child's torso and supports their back. Although it is use primarily for scoliosis, it can be used to treat children and adults who are recovering from back surgery or have suffered back injuries. This type of brace is made of durable place specifically molded to fit their body.

How it works

The TLSO back brace works by exerting pressure on different points along their spine. Over time, their spine will give way to the pressure of the back brace. When this happens, the curvature of the back will slowly start to straighten.

How it is worn

The TLSO back brace is generally worn under their clothing so it is essentially invisible to other people. When this brace is put on, it stays on up to twenty-three hours a day. During strenuous physical activity it is generally removed. It is worn when they sleep.

Material they are made from

The child's version of the TLSO back brace are generally made of plastic but if an adult has to wear one they can be made of different materials and in different styles. Some of these are made from a mesh-like fabric. This gives their body a breathable, cool feeling. They can also be constructed of plastic that is more malleable to give the one wearing it more freedom in twisting and bending. If they only need a low level reduced pressure on their spine and have a low level of restriction the TLSO brace may be made of elastic and foam.

Guidelines

There are some universal guidelines when this type of brace is worn.

• Some type of shirt made of a lightweight material is worn underneath the back brace to soak up sweat and enhance comfort.
• You will have to keep an eye on any type of irritation or skin rashes that the child may develop from wearing the TLSO back brace. These need to be reported to the physician and treated before it becomes worse.
• The child never wears it while taking a bath or shower. The doctor will have a special brace they need to wear in the shower.
• When sitting in a chair it should be a sturdy one with arms.

Back Pain: Why Keep Suffering?

Mr. Sharma had been an active person all through his life. But from his 65th birthday, he started having difficulty in walking. He would easily get tired and his legs would feel heavy. Tingling and numbness, with leg pain had made his active lifestyle a thing of the past. Gradually he became afraid of walking. It was becoming increasingly difficult for him to carry out his daily chores. The thought of being dependent on others, finally made him visit a doctor. He underwent a few tests and MRI of the spine. He was diagnosed with Lumbar canal stenosis, a condition which causes abnormal narrowing of the spinal canal carrying the nerve roots to the legs.

He underwent a minimally invasive surgery for the spinal stenosis. After surgery his world had changed. Next day he could walk and could again feel the nimbleness in his feet. He had no pain and he again started touring of travels, he had long wanted to do. The incision was very noticeable. Three months after the surgery, he regrets only one thing. Why did he go to a doctor so late? Perhaps, he was afraid of the surgery. He had heard of so many patients not recovering from back surgery, having persistent pain or being bedridden forever.

Spine surgery underwent massive developments over the last decades. The results of surgery have improved drastically. Still, there is a lot of fear of spinal surgery. This is mostly because of lack of awareness about spinal conditions and their treatment. Some pain can have simple answers like physiotherapy and slight models in lifestyle. Only very few of back pain patients require surgery, perhaps less than 1%. With the advances in spinal surgery, especially minimally invasive or keyhole spine surgeries, the mental and physical trauma during recovery phase has also been reduced drastically. After all these facts, there is so much of apprehension surrounding spine surgery. Many patients do not even visit a doctor because of such ill found fears. This needs to be addressed through media campaigns.

So, if you have been ignoring a back problem for a long time, it is better to visit an expert. Imagine a life free of pain, and stop suffering. Do not worry even if you have to undergo a procedure also, and believe in your doctor. Very few patients have recurrent problems after back surgery, and high chance are you may not fall in that category.

What Do Chiropractors Do and Why?

Chiropractic has been around for over a century and was originally defined by its creator DD Palmer and taken from the Latin for 'done by hand'.

While it took some time to develop a basic premise, in the early stages the ideas that formed its base had many impacts. DD Palmer was originally a 'magnetic' healer with an interest in the metaphysical. BJ Palmer his son had probably the most impact on the expansion of the profession.

Hippocrates said “It is most necessary to know the nature of the spell. One or more vertebrae may or may not go out of place very much and if they do, they are likely to produce serious complications and even death, if not properly adjusted. Many diseases are related to the spine “.

This may have been an under thought that people at the time had. DD could have, at least at some point believed it to be always the case. And so he set about 'adjusting' or manipulating the spelling in an attempt to help people with various complaints.

The basis of care focused on the integrity of the spinal and nervous system. It was known that the brain controls every system in the body and uses the spinal cord and nerves to deliver messages to every part of the body. Palmer taught that if you are unwell there is a good chance that your nervous system was involved and that the chiropractor's job was to correct it.

He used his metaphysical interest to introduce the concept of 'Innate intelligence'. This thought involves the inbuilt ability of the body to heal itself and that any interference to the “flow” of this intelligence could cause 'dis-ease'. “The kind of dis-ease depends upon what nerves are too tense or too slack”

Later that concept developed into a more accepted version. This was still based on the thought that the body has an inbuilt ability to heal but that stresses on the body would disrupt the spinal joint motion. The most common causes of stress was believed to be Emotional, Physical and Chemical.

By the 1920's the research on spinal motion, disc herniation and nerve impingement was well under in the medical realm. This at least showed that disruption to a spinal joint can irritate nearby nerves. DD termed this a Vertebral Subluxation – a dysfunction of a spinal joint that irritates a spinal nerve. It is a chiropractic specific term not to be confused with the medical term of subluxation.

And it has been this term 'vertebral subluxation' that has continued to dog the profession from inside and out. From inside there are practitioners who hold firm to this construct, others that have let it go, each side being very vocal. Of course from outside the profession this has been seen as a mark of disunity and has tarnished the professions credibility.

Palmer said, the chiropractor's role is to adjust subluxated spinal vertebrae back to their best position and restore joint motion. The promise being that this will remove any nerve interference and associated symptoms.

This is what they do, and the reason they believe they do it is of some contention. But what is not at all debated is that Workcover NSW Australia (a state-wide workers compensation scheme) does not have any bias toward Chiropractic or Physiotherapy treatment for injured workers. This at least implies that they have similar return on investment to each other with respect to injured workers regardless of the type of injury.

A study in Canada in the late 1990's associated the results from chiropractic and other modalities for the treatment of lumbar disc herniation was effective and appropriate in comparison to a medical alternative treatment. A number of other studies also stated that manipulation is safe and appropriate for the great majority of patients with lumbar disc herniation.

So it appears that this constantly debated promise for why chiropractor's do what they do has very little bearing on patient exit.

A recent study out of New Zealand 2016 shows that a manipulation does in fact cause a change in brain function. So perhaps the research will one day be able to clarify the underlying belief of the subluxation that has dogged and divided the profession.

Whether a Chiropractor is improving the “Innate Intelligence” or helping the lower back repair after a work injury, one thing is still a reality, they have been helping people naturally for more than 100 years without drugs or surgery. Maybe this could be a more appropriate fact to inspire the profession to unite and move forward?

How Important Is Chiropractic Care If You Have Been in a Car Accident?

Once major traumatic injuries such as broken bones, lacerations, contusions, and internal injuries have been ruled out by the EMTs it is equally as important to look at possible problems which have occurred in your musculoskeletal system. (Soft tissue damage and misalignment issues).

Often times following an accident your body may be in shock. It is producing dopamine and other chemicals in the brain in order for you to be able to function and you may not feel the damage, pain or soreness yet.

Although pain following a car accident can be felt anywhere in the body, the most common are neck injuries due to the way we are anchored into our vehicles . Although we are strapped in by the body, our heads have to remain mobile. When a car or truck is hit, your neck is whipped in one direction then back the other way. Hyperflexion-hyperextension injuries can affect the soft tissues in the neck as well as intervertebral discs and joint capsules.

Whiplash can be the result.

Whiplash may present itself in the following ways:
Neck pain
Arm pain or numbness
Inflammation

Nausea
Feeling like you may pass out
Lack of clear thought
Anxiety
Jaw issues
Back pain
Even Depression can be a side effect from a whiplash injury.

Chiropractors are musculoskeletal specialists. Seeing one as soon as you are able to following an accident can help you to heal as expediently as possible while minimizing damage. They will perform a thorough exam and physical assessment including X-rays. If they feel you will benefit from an immediate adjustment they will do so at that time as well as set the best course of treatment going forward. After even a minor accident, the curvatures in the spine are straightened, especially the neck or cervical spine. The soft tissues may heal (often with scar tissue), but the spinal curvature remains straightened. Over time, this may cause osteoarthritis and degenerative joint disease. – Arnica Montana – A homeopathic for bruising and muscle soreness may be beneficial as well. It is taken sublingually every 2-4 hours until symptoms begin to subside.
-Meditation has also been known to help alleviate pain, at least temporarily and can offer some relief.
-Rest helps to promote healing.
